This is not really an Application. This is just a way to let us know who you are, if you have the intention to complete the full training program. This is not a commitment, just a way of introducing yourself.

Note about the professional license: You may be eligible to attend some or all the courses if you are in a related field, but you need to be a therapist to receive FOT certification.
